1. Unpack it to create a folder caller "wheel"

2. You can rename it to anything you want.

3. Upload the entire folder onto your server 

4. Ensure the folder named "cache" has permissions of 775

5. Visit this URL: http://yourserver.com/wheel/wheel.php to run the script
